Required Forms

Listed below is a summary of required forms for the School Psychology program followed by pertinent information and specific prerequisites. Unless otherwise noted, all forms may be obtained from the Department of Special Education, Rehabilitation and School Psychology Office in Room 316 of the Education Building.

 

Form Description
Advancement to Candidacy

Complete no later than the semester prior to graduation.

Each student working toward a Master's degree must file an application for Advancement to Candidacy, indicating a proposed program of graduate study. This procedure should begin as soon as the classified graduate student has completed the following:

  • Removed any deficiencies in Admission Requirements
  • All Degree Requirement Prerequisites
  • At least 24 units of 200 level coursework with a 3.0 GPA before filing advancement
  • Successfully met the University English Writing Proficiency requirements

Advancement to Candidacy forms are available in the Department office and the Graduate Center. You should complete the form after planning a degree program in consultation with your faculty advisor.

Certificate of Clearance / Credential

Submit during your first semester in the program.

This certificate is required before you will be allowed to work in the schools. Therefore, it is important to begin this process as soon as you enter the program. The form is available in the credential office, Eureka 209. Phyllis Stenman and Carol Lucido are credential analysts for School Psychology. This form must be submitted before enrolling in Field Study

Petition to Enroll in EDS 440

Practicum in Individual Counseling/School Psychology
(submit before enrollment in EDS440)

Practicum in Individual Counseling/School Psychology: This form must be submitted prior to enrollment in EDS 440 - Practicum in Individual Counseling/School Psychology. In addition to having the following courses completed before enrollment in EDS 440, the School Psychology student must provide evidence of professional liability coverage.

Prerequisites:

  • EDC 216 - Basic Counseling Theory
  • EDC 280 - Practicum in Communication
Petition to Enroll in EDS 441

Internship A (first semester)
(submit one semester before enrolling in the first semester internship)

This form must be completed one semester before enrolling in the first semester of Internship. In addition to having the following courses completed before enrollment in Internship A, The School Psychology student must also have professional liability coverage.

Petition to Enroll in EDS 441

Internship B (second semester)
(submit one semester before enrolling in the second semester of Internship)

This form must be completed one semester before enrolling in the second semester of Internship. In addition to having the following courses completed before enrollment in Internship B, The School Psychology student must also have professional liability coverage.

Petition to Enroll in EDS 540/541/249

Masters Thesis/Project/Oral Exam

(Submit one semester before enrolling in your culminating experience. You must first contact a faculty member to be your thesis/project advisor if you will be enrolling in EDS 540 or 541. Student must file a reservation form signed by their Sponsor EACH semester to register for the Thesis/Project.)

Program Planning Sheet for School Psychologist Credential

Submit with the School Psychologist Credential Application

This form lists School Psychology Coursework/Equivalencies. The date the course requirement was met, units completed, and identification of any waived or equivalent courses must be completed. After the Program Planning Sheet is approved by the student's School Psychology Advisor, it is to be attached to the School Psychologist Credential Application.

School Psychologist Intership Credential Before you begin your internship you will need to be approved for an Internship Credential. This process is handled through the credentials office, Eureka 209. Requirements for receiving your Internship Credential are outlined in the Fieldwork and Internship Manual. You will need to have completed your course work, passed the credential exam and have approval of all program faculty before you receive your Internship Credential.
Credential Applicaiton

Available in EUR 209 when approved for Internship and Credential

This credential is also administered through the Credentials Office, EUR 209. Credential applications are available in the office at the end of your internship year for students who have been approved for the credential. The program planning sheet for the credential is filed with this application.

Application for Graduation

Submit by November 1 for Fall and March 1 for Spring or Summer graduaiton

These forms are available in the Department office and the Graduate Center. Submit Application to the Graduate Center by November 1 for Fall and March 1 for Spring or Summer graduation. You must have an approved Advancement to Candidacy form on file in Graduate Studies before submitting your Application for Graduation. For more information, contact Graduate Studies at 278-6470.
